We are open to serve you, answer questions or schedule an appointment during the following hours:

Monday – Thursday: 8 am– 5 pm (Closed for lunch 12:30-2.)
Friday: 8 am – 12 pm (for emergencies)

(Summer Hours for June 14 - August 20, 2010 are Monday - Thursday: 7:30 am - 4 pm.  Fridays 8 am - 12 pm. Closed for lunch 12-1.)

Appointments
Your scheduled appointment time has been reserved specifically for you. We request 24-hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment. After missing your second appointment without notifying us 24 hours in advance, you are subject to being charged an additional fee.  You may check for any scheduled appointments by going to the "Patient Login" on the top of this page, and entering the User ID and password you have been sent by this office.  Changes in appointment times can only be made over the phone.

Insurance
Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. We ask that you not assign payment of insurance benefits to our office.  Benefits will usually be sent to the policy holder.  Payment for the initial consultation is due at the time of the appointment.  After that, payments will be due according to an agreed upon schedule.

As a service to our patients, we will complete the insurance form you will need to submit for a pre-authorization for payment for services.  Your insurance company will then send any payments to you as reimbursement for the fees you pay directly to our office.
